Advance Care Planning is not about choosing, it’s about preparing for healthcare decisions.

Join us in learning to identify your Substitute Decision Maker (SDM) and the steps of planning and developing your advanced care directives to ensure all the people who may be responsible for your care will know what you want to have happen.

Participants will receive a detailed step by step workbook. Questions and discussions are welcomed with the freedom to share values, wishes and beliefs that guide your personal future care.
